Not All Scars Are Surface Level
Acne scars and facial indentations are not always surface level texture changes. In many cases, the skin is being pulled downward by tight bands beneath it.
When tissue is tethered in this way, facial movement creates a visible tug of war between muscle activity and the anchored area. This can make the surface appear indented, shadowed, or uneven in certain lighting.
Subcision is a minimally invasive technique used to release this tethering. Once released, the skin can move more freely and remodel more evenly over time.

How it Works
1. Consultation and Scar Assessment We evaluate scar type, depth, and skin movement to determine whether tethering is present and whether subcision is appropriate.
2. Targeted Scar Release A specialized instrument is placed just beneath the skin to release areas that are pulling downward. This allows the skin to settle more naturally and reduces visible shadowing.
In select patients, releasing tethering can create a more natural improvement than adding volume alone. Treatment may be performed with or without filler support depending on anatomy and goals.
3. Gradual Remodeling Improvement develops over time as the skin heals and collagen rebuilds. Subcision is often combined with PRP, Sculptra, or microneedling to support long term tissue remodeling.
