5 Areas You Should Never Get Sculptra®

1. Around the Eyes (Under-Eye Hollows or Tear Troughs)

The eyes are often the first place we notice aging. Hollows, dark circles, or thinning skin around the eyes can make us look more tired than we feel. It’s natural to wonder if Sculptra® might be the answer.

Unfortunately, it’s not the right tool for the job here.

Why? The skin under the eyes is incredibly thin and delicate. Sculptra® works by stimulating collagen, but in this area, the risk of lumps, nodules, or irregularities is just too high. Because Sculptra® takes months to build collagen, there’s no way to “erase” a problem quickly if something goes wrong.

Better alternatives: Hyaluronic acid (HA) fillers are usually preferred for under-eye rejuvenation because they can be precisely placed and—if necessary—dissolved. Treatments like laser resurfacing or medical-grade skincare can also help with under-eye texture and pigmentation.

Our advice: If under-eye hollowness is your main concern, skip Sculptra®. We’ll guide you toward options that are safer and more predictable in this delicate area.

2. The Lips

It might sound surprising, but yes, sometimes patients ask if Sculptra® can plump lips. The answer is a firm no.

Lips are made up of soft, highly mobile tissue. Sculptra®, by design, is not a filler, it’s a collagen stimulator. If placed in the lips, it could cause hard nodules, uneven texture, or even interfere with natural lip movement. The result would be uncomfortable at best, and unnatural at worst.

Better alternatives: HA fillers (like Juvederm or Restylane) are the gold standard for lips. They provide immediate results, are soft and flexible, and can be adjusted to your preferences.

Our advice: Keep Sculptra® away from the lips. Use it where structure and support are needed, not where softness and flexibility are key.

3. Forehead or Between the Brows (Glabella)

Sculptra® is also a no-go in the forehead and glabella (the space between your eyebrows). These areas are highly vascular, meaning lots of blood vessels run through them. Injecting Sculptra® here carries a higher risk of complications, including rare but serious issues like vascular occlusion.

On top of that, the forehead muscles are very active. Constant movement in this area makes it difficult for Sculptra® to settle evenly, raising the risk of lumps or asymmetry.

Better alternatives: Neuromodulators like Botox, Dysport, or Xeomin are the safest and most effective treatments for forehead lines and frown lines. If volume loss in the temples is contributing to a hollowed look, Sculptra® can be used safely there, but not in the forehead itself.

Our advice: For smoother brows and a refreshed upper face, stick to Botox-type treatments. Reserve Sculptra® for mid-face and lower-face support.

4. Hands

This one can be confusing, because yes, volume loss in the hands is a common concern, and yes, some fillers are used safely in this area. But Sculptra® is not one of them.

The hands are bony, tendinous, and in constant motion. Sculptra®’s collagen stimulation process can lead to lumps, bumps, or uneven results when used here. Plus, since our hands are always moving and exposed, any irregularities would be noticeable.

Better alternatives: Radiesse (a calcium-based filler) and certain HA fillers are FDA-approved and specifically designed for hand rejuvenation. They provide smoother, more predictable outcomes.

5. Around the Mouth (Especially the Vermillion Border)

The skin around the mouth is constantly in motion, talking, eating, smiling, and Sculptra® doesn’t do well in such dynamic areas. Placing it here risks lumpiness, nodules, or uneven collagen stimulation that could make lines look worse instead of better.

Better alternatives: Tox “lip flips,” HA fillers for subtle smoothing, and laser or microneedling treatments are better suited for these fine perioral lines. Skincare with retinoids or medical-grade peptides can also help maintain long-term results.
